Thank you for your interest in supporting our service users with a range of health needs, upon discharge from West Suffolk Hospital.
This service focuses on supporting the needs of NHS staff and outside agencies, by helping service users (patients) work towards becoming independent where possible upon discharge.
Service users may be dealing with a variety of issues, impacting negatively on their well-being, such as long-term health conditions, Infections, poor mental health, finance issues or substance misuse. Working alongside ACT Support workers & Coordinators or meeting face to face with patients the volunteers will be assisting in the support of a person centred, holistic, support plan.
Successful support in the community can alleviate hospital admissions and readmissions; therefore, increasing the number of available hospital beds and resources available to the NHS. This is a great role for somebody who is empathetic and passionate about supporting Service users in the community upon discharge from a medical setting. It would also suit somebody who would like experience in a health and social care support role within a range of diverse settings, including individuals’ homes and the local community.
You can be involved in any or all of the following ways:
• Providing a listening ear for general conversations around wellbeing, offering emotional support in the form of befriending.
• Assisting in arranging appointments, including GP and hospital, and supporting them to attend.
• Helping with shopping, or prescriptions if needed, whether that be assisting in online shopping or collecting /arranging delivery for shopping/ prescriptions.
• Helping to arrange practical support, enabling them to maximise their independence at home and within the community, such as helping them to find longer term support in the form of cleaners or dog walkers.
• Identifying and signposting the individual to any relevant services /activities and support groups.
We DO NOT assist with any Personal Care or Medication input.
Our Volunteer team is drawn from a range of backgrounds, with an equally wide range of life experiences: we welcome all applications even if you feel you have no relevant professional experience, if you are the right person with the right mind set, we will support you through training, shadowing and support sessions.
We have a fully comprehensive, free training package, which includes:
- Professional Boundaries
- Equality, Diversity & Inclusion
- Safeguarding – Adult & Child – Level 2 accredited
- Mental Health Awareness
- Criminal Exploitation of vulnerable Adults and Children
- Supporting health and social care needs in the community
- Shadow sessions
Any and all mileage expenses are covered at 45p a mile.
